We integrate a range of models and methods to cater to the diverse expertise within our team, which includes both emerging and seasoned care workers and nurses from various cultural backgrounds. This inclusive approach has prompted us to dynamically redefine roles to ensure optimal use of our resources, particularly during staff shortages and the challenges presented by the pandemic.
In line with the CQC’s “Right Care, Right Support, Right Culture” guidance we have enabled Health Care Assistants to carry out designated nursing tasks under the supervision of registered nurses, thereby expanding their skill set within our care homes. This empowerment has led to the promotion of some Health Care Assistants to unit manager positions, in recognition of their leadership and clinical competencies.
Our hybrid model, which is applied within our registered care homes, is designed to address the varied skills and staffing requirements, as well as the personal needs of our service users. It upholds a high standard of care and incorporates a customised version of the Roper-Logan-Tierney Model of Nursing, enriched with elements from Functional, Team, Total Care, and Primary Care models. This blend ensures a balance between task-focused and person-focused care.

We also embrace Dorothy Orem’s self-care deficit theory, which encourages activities that individuals can undertake independently to maintain life, health, and well-being. What care models and theories does Trust Care use?
We use a hybrid approach that integrates several evidence-based care models. These include the Roper-Logan-Tierney Model of Nursing (which structures care around 12 activities of daily living), Dorothy Orem’s self-care deficit theory (which identifies where a person needs support and where they can be independent), and Florence Nightingale’s environmental theory (which emphasises optimising the person’s physical environment to support their recovery and well-being). We blend these frameworks so that care is both clinically structured and genuinely personalised.
How does Trust Care empower residents in their own care?
Empowerment runs through everything we do. Residents are actively involved in decisions about their daily lives and their care plans. We use a Named Nurse and Key-Worker system so each person has a consistent relationship with someone who knows them well. We promote independence by supporting people to maintain and develop skills rather than doing everything for them. Where appropriate, we also enable Health Care Assistants to take on expanded clinical responsibilities under nurse supervision, which has led to internal promotions and better continuity of care for residents.
How do you maintain care quality during staff shortages?
We’ve developed a flexible staffing model that allows us to adapt roles based on need. During the pandemic and ongoing workforce pressures, we enabled experienced Health Care Assistants to carry out designated nursing tasks under the supervision of registered nurses, in line with CQC guidance. This expanded their skill set and improved continuity of care. Some HCAs have since been promoted to unit manager positions. We also invest heavily in in-house training so that all staff, regardless of background or experience level, understand and can deliver our care approach consistently.
What does your in-house training programme cover?
Our training begins during induction and is designed to connect theory with practice. New staff learn our hybrid care approach, including the specific models and frameworks we use, and how they apply in real situations within our homes. Training continues through mandatory and statutory refreshers plus a CPD (Continuing Professional Development) programme. Staff without previous care experience are supported through the National Care Certificate during their probationary period. We actively encourage staff to pursue additional national qualifications throughout their career with us.
